Step 7.  Fill out the Team Registration Form.  (Sample)

Step 8.  Fill out the Team Bond Form.

Step 9.  Compile the packet to send to the Registrar.

a.)  The Team Bond Form, completed, with check attached, payable to SURVA. ($150/team to maximum of 5 teams for a total of $750)

b.)  Team Registration Form with a check attached, payable to SURVA, for the fees calculated for the team.

c.)  Any required Background Screening Consent Forms for the adult members of the team.

d.)  A copy of the FIRST PAGE ONLY of each members online application confirmation email. Please attach them to the team registration form in the order they are listed on the form.
