Registrations 2007 for Juniors Programs

Note: Juniors programs (clubs and teams) should use the Prospective Member Information Form when holding tryouts.  This is NOT a registration form and is non-binding between the club and the participant. This provides protection during the tryout.  Do not send these forms to the Registrar as all registration is on-line.

The electronic waiver acceptance process is now in place for all members as a part of online registration. To verify identification of the individual legally accepting the terms of the membership, a driver’s license or state identification number must be entered.  It is imperative that a parent or guardian participate in the registration process for junior players because the person agreeing to the Waiver & Release of Liability  and Code of Conduct must have legal capacity and authority to act for and on behalf of the minor.

Club Directors

Step 1.  Create a club or update your current club information for the 2007-2008 season using the Club Form. Fill out the Club Form, save it to disk, and email it to the Registrar. When that information is updated to the USAV site you will receive a confirmation email. You may then direct your members to register individually online.  In order for your members to associate with your club, your club must be listed online.  Please stress to your members to choose the correct club when registering.

Step 2.  Print the Volunteer Waiver Release and have each adult registering with the club sign one. Retain these records.

Step 3.  Print the Chaperone Responsibilities Form and give to each chaperone in the club and have them sign it.
